Kiel Ciasca is an orthopedic and sports physiotherapist in Campeche, Florianópolis. He treats athletes, chronic patients and expats, and holds a 5.0 rating with 71 Google reviews. His old site was a static business card: no technical SEO, no English version, no tracking. Patients only arrived by referral. The brief was short: "I want people to find me on their own, in Portuguese, in English, and when someone asks ChatGPT."
The challenge
All the reputation lived inside the clinic. Anyone searching "physiotherapist in Campeche" for the first time found nothing. And Florianópolis is full of tourists and expats who search in English. The site had to show up in both languages and in AI answers, not just word of mouth.
What we built
Separate PT and EN routes, correct hreflang, LocalBusiness and MedicalClinic schema. Service pages with semantic FAQs and short, direct answers, the format ChatGPT and AI Overviews cite. Real photos shot at the clinic, no stock, with LCP under 1.8s on 4G. WhatsApp as the primary conversion, with a pre-written message on every button.
The result
In 90 days: 1.1K views with no meaningful paid media, 128 direct sessions and visitors from 7 countries, from Brazil to Canada. The 5.0 Google rating held. New patients arrive already knowing what the clinic does.
